Hello! Thank you for taking the time to fill out this questionnaire. We are so excited that you are considering one of our amazing puppies! This short questionnaire serves 2 purposes. It helps us know if you are a good match for owning a doodle, and it also makes you aware of some of the needs of a doodle--maybe some things you haven't yet thought about. I realize that these questionnaires may seem a little daunting, but please answer honestly, and know that we are just trying our best to ensure that our puppies are going to the best homes possible!

- Doodle mixes require professional grooming approx. every 2-4 months and it can cost $75+ each time. Lack of grooming can cause serious skin problems. Are you committed to this necessary expense?*

- Do you agree to contact us in the case that you need to re-home this puppy for whatever reason?*
- Completing this application is a step in the process to acquiring a puppy from Sadie's Doodles, but it does not guarantee you'll get a puppy from this or any future litter. While it's unlikely, the breeder reserves the right to refuse any placement at any time.
  
  APPROVAL AND DEPOSIT: I will review applications within 2 business days of receipt. Applications are processed in the order they are received. If your home is a good fit, I'll notify you. We breed infrequently, so we will only accept deposits when puppies are expected or actually available.
  
  YOUR PICK OF PUPPY: Puppy pick week is week 4-5. We ask that you make this decision by this time so we have adequate time to find the perfect homes for the remaining puppies.
  
  GOING HOME: We cannot hold puppies for extended times; it is critical for their social development that the litter departs within a reasonable timeframe. If you have a big vacation planned or have events that would conflict with a young puppy, please know puppies cannot be boarded in a kennel until they have received their full immunizations and their rabies vaccination which cannot be done until 3 months of age.
  
  CONTRACT: We have a contract and will not place a puppy without a signed contract. If you have questions about specific terms, you need to contact us prior to picking up your puppy.
  
  SPAY/NEUTER: We require spay/neuter at or before 12 months.
  
  REGISTRATION PAPERS: Our puppies come with an option to register them via CKC.
