I am the person whose name appears on this form and all information is correct to the best of knowledge. In addition, I understand that EOS House of Healing and Recovery, INC (hereby referred to as “EOS” is a luxury service, not a medical service or medical provider) intended to provide a comfortable and private setting to assist with mental and physical recovery following outpatient cosmetic surgery procedures. I understand that EOS is not a substitute for medical care or hospitalization and, as such, I am electing to use the services of EOS House of Healing and Recovery, INC as a matter of convenience and not for reasons related to medical care. I hereby acknowledge that I am of legal age and of sound mental capacity and that I make this election free from duress or undue influences.
I understand that all services offered and/or provided by EOS House of Healing and Recovery, INC are provided exclusively by EOS and are not provided by any of its or their affiliates or employees without limiting the foregoing.
Financial Responsibility
I agree that my liability for this bill is not waived in the event that the indicated person, company or association fails to pay for the full amount of the charge.
Check-In & Check-Out
Our check-in time is 3 pm and our check-out time is 12 pm. If you need to check in early or check out late, there is a surcharge and it is based upon availability. Please plan your flights accordingly.
- I understand that if early check-in is available, I will be charged a fee of $50 to have access to a bed. Once check-out time approaches, I understand that I will need to leave the premises or pay a late checkout fee, not exceeding $50. There is a $30 fee to stay in the common areas and a $50 fee to stay in a room (based on availability). Late check-out does not include transportation to the airport and I understand it will be my responsibility to transport myself to the airport any time after 1:00 pm.
- I understand that check-in time closes at 9:00 PM and that I will not be granted access to the facility after this time.
- I understand if I check-in after surgery, I am not entitled to a bed until check-in time which is 3:00 PM. EOS makes every effort to accommodate your needs however a guest cannot be removed from their bed prior to 12:00 PM.
- I understand that if I check in after surgery there is NO guarantee that a bed will be available prior to check-in time and EOS is unable to store my luggage in the event that the surgery center will not allow me to bring it into their facility.
Meals
Eos House of Healing and Recovery does not provide meals in accordance with the State of Florida statutes and guidelines. Snacks and beverages outside of water is complimentary and should be consumed within reason. Should our complimentary snacks and/or beverages become unavailable, you agree to purchase your own snacks and beverages at your expense.
Airport Transportation
EOS airport transportation is only for Miami International Airport.
- I understand that EOS offers shared transportation service, and does not offer private car service, wait times can be up to sixty minutes.
- Eos does NOT transport pickups from Hotels, Airbnb, or any alternative locations.
- Transportation is reserved solely for patient use and is for transportation to Miami Airport and Surgery Centers.
- If your flight time surpasses check-out by more than one hour (1:00 PM), EOS will not offer transportation to the airport.
- I understand that airport pick-up times begin at 8:00 AM and end at 4:00 PM.
- I understand that airport drop-off times begin at 8:00 AM and end at 1:00 PM.
- Please note, Eos House of Healing does not offer transportation on Sundays and we do not allow check-ins after 9:00 PM. If your arrival time is outside of the days and times, we do offer transportation, you are responsible for absorbing the cost of transportation to our facility.
Medical Risks and Responsibility
EOS is not a medical facility and is a service to facilitate my recovery, monitor my recovery, and follow my Doctors discharge instructions.
- Further, I understand that my medical care, at all times, and in all circumstances remains the responsibility of the surgeon who performed my surgery.
- I understand that only my surgeon is authorized to write prescriptions for me and to determine whether outpatient recovery care is appropriate for my particular recovery needs.
- I understand that my surgeon is the only party with authority to order and perform post-operative medical treatment and he or she reserves the right, alone, to order my hospitalization, if that is the level of care I require.
- I further understand that my surgeon is the only party authorized to determine the level of post-operative care needed following my procedure, including whether discharging me to a non-medical facility, such as home or EOS, is appropriate.
- I understand that I may be unsteady on my feet following surgery and I agree to request assistance getting out of bed for the first 24-hrs following surgery. In addition, while on pain medication, I agree to remain in my recovery suite at EOS unless accompanied by a friend, family member or EOS nurse.
- In addition, I fully understand that it is MY responsibility to check with my surgeon, prior to my surgery date, for verification on which over-the-counter AND prescription medications I may take while recovering at EOS.
- I agree to ONLY bring over-the-counter & prescription medications, to EOS, which my surgeon has verified I may safely restart on arrival to the facility. I understand that it is my responsibility to place ALL approved medications, BOTH prescription and over-the-counter, in ONE MEDICATION BAG and store it in the safe provided by EOS. EOS does not administer medication and is limited to reminding you about your medication.
- Furthermore, I hereby agree to indemnify and hold harmless EOS and all of its owners, agents, officers, directors, employees and affiliates from and against any and all loss, liability, damage, cost, expense (including reasonable attorneys’ fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whosoever that I may sustain in connection with choosing not to comply with EOS’ medication policy stated here.
- I understand that I elected to have an outpatient procedure which means I was released to Eos, a non-medical facility through approval from my Surgeon. Eos is not a medical facility and cannot wipe your genitals, shower or bathe you, wash your hair, or provide personal care. To be cleared for surgery you must be healthy and of sound mind. While you may exhibit symptoms of soreness and discomfort, you should be able to walk, use the restroom, and shower. If you determine that you cannot perform these tasks, Eos will contact 911 on your behalf. Your inability to perform these tasks may be a sign of complications from surgery.
Medications & Prescriptions
Rx pickups, injections, and additional medical supplies costs are absorbed by you. Any additional medical supplies will accrue a fee.
- We require you to bring cash to purchase your prescriptions. If you choose not to provide cash for your prescriptions and your method of payment fails, it is likely your prescription will be picked up the following day.
- Prescription drop-off concludes at 5:00 PM and prescriptions will be taken to the pharmacy the following morning. The only exception is if the prescription was given on the day of surgery.
- Eos Drivers are only permitted to go to Walgreens to fill prescriptions. This is to avoid any delays with filling your prescriptions. You will have to provide your own transportation to an alternate pharmacy should you choose to decline the services at Walgreens Pharmacy.
- I understand it may take up to 8 hours to fill my prescriptions and EOS has no control over the given times at the pharmacy.
- Again, I agree to ONLY bring over-the-counter & prescription medications, to EOS, which my surgeon has verified I may safely restart on arrival to the facility.
- Again, I hereby agree to indemnify and hold harmless EOS and all of its owners, agents, officers, directors, employees and affiliates from and against any and all loss, liability, damage, cost, expense (including reasonable attorneys’ fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whosoever that I may sustain in connection with choosing not to comply with EOS’ medication policy stated here.
No Smoking Policy
I understand that EOS has a strict NO SMOKING policy and I will be fined $500 if found smoking in my room. I understand that smoking causes significant detriment to my body’s ability to heal and greatly increases my risk of post-op complications. I hereby agree NOT to smoke while at EOS.
- I also agree NOT to request the assistance of an EOS staff member, hotel employee or any other person to assist me outside to smoke.
- Should I decide to smoke and breach this contract, I am doing so at my own risk. I hereby agree to indemnify and hold harmless EOS and all of its owners, agents, officers, directors, employees, and affiliates from and against any and all loss, liability, damage, cost, expense (including reasonable attorneys’ fees and disbursements, and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whosoever that I may sustain in connection with choosing to smoke while at EOS.
Transportation
I hereby assume all risk and personal injury, sickness, death, damage, and expenses as a result of participation in transportation activities involved with EOS. Further, authorization and permission are hereby given to EOS to furnish any necessary transportation, and I hereby agree to hold harmless and indemnify EOS and all of its owners, agents, officers, directors, employees, and affiliates for any liability sustained by services provided by EOS as a result of the negligent. Willful or intentional acts of EOS including expenses incurred attendant thereto and hereby give permission to hospitalize and hereby authorize medical treatment, including but not limited to emergency surgery or medical treatment and assume the responsibility of all medical bills if any. I understand that this is a legally binding release and consent.
- I have carefully read this Release of Liability and Consent for Transportation and Medical Treatment policy, outlined here and fully understand its contents. Being aware of said contents, I sign of my own free will.
- I understand that whenever myself, a family member or friend will be transported, that I hereby release and hold EOS and all of its owners, agents, officers, directors, employees and affiliates harmless from any and all loss, liability, damage, cost, expense (including reasonable attorneys’ fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whatsoever that I, a family member or a friend, may sustain in connection with the services provided to me by EOS or any of the foregoing.
- Your stay with us includes transportation for two post op appointments. If you exceed two post op appointments, there is a fee associated with transportation and is subject to availability.
- We do not allow outside massage therapist to enter Eos. Only our approved vendor may service our facility. If you scheduled with an outside massage therapist, you are responsible for your own transportation to those appointments.
Cancellations
In order to provide the best experience possible for our guests, if you need to cancel your stay with us for any reason, please email us as we need the cancellation in writing.
- Yes I understand that my deposit is non-refundable and non-transferable and I must cancel at least 30 days before my check-in date to receive 50% of any additional monies paid.
- No changes can be made to your reservation once you check in. If you must depart early, you forfeit your deposit and all monies paid.
- If your full balance is not paid within 7 days of check-in you must contact us to make a payment or your reservation will be canceled and you forfeit any monies paid. If we do not receive prior notice based on our cancellation policy you will forfeit your deposit or any monies paid.
- You must submit all required forms, financial obligations, and provide flight information at least 7 days prior to your arrival or we reserve the right to cancel your reservation and you forfeit all monies paid.
- We are unable to plan for your stay or transportation without your flight details.
Loss & Damage
Loss & Damage EOS House of Healing and Recovery, INC shall not be liable for any lost or damaged items during your stay. EOS House of Healing and Recovery, INC recommends that you bring any personal property at your own risk. If you should leave any item (s) behind, you are responsible to pay the postage to return those items and schedule pick up from your carrier of choice. Eos House of Healing and Recovery is unable to ship any items left behind during your stay.
Code of Conduct
Eos House of Healing is a Recovery home and we promote an environment that is conducive to healing. We require that you speak in a low tone while within the facility and be considerate of other guests and our staff. We do not allow any profanity inside or on the premises of our facility. We require that you are respectful towards all guest and our staff at all times. Additionally you must use head phones while on FaceTime or any video call. We do not allow the use of speaker phone in order to preserve the peace of our other guests. Failure to follow this code of conduct will result in removal from our facility and you will forfeit all the monies paid.
We do not permit any guest to urinate or defecate on the floor in any part of the house. If you violate this policy you will be required to vacate the premises immediately and will not be entitled to a refund.
We do not permit our guest to flush gauze, baby wipes, or compression stockings down the toilet. This causes stoppage in the facilities plumbing and affects the usage of the bathrooms. If it is determined that our guest violate this policy, you will be required to vacate the premises and will not be entitled to a refund.
We do not allow any abuse whether verbal or physical of our staff. This policy has a zero tolerance and includes but is not limited to physical touch, spitting, kicking, and intentional release of secretions. Eos House of Healing and Recovery INC reserves the right to reasonably determine what constitutes as abuse. If you violate these terms you will be required to vacate the premises and forfeit all monies paid.
NON-DISPARAGEMENT
I agree that I shall not, at any time during the Term of this Contract or thereafter, make statements or representations, or otherwise communicate, directly or indirectly, in writing, orally, or otherwise, or take any action which may, directly or indirectly, disparage Eos House of Healing and Recovery INC, its affiliates, staff, employees or its contractors.
In consideration of the foregoing, I still elect to utilize the services of EOS. I agree to indemnify and hold harmless EOS and all of its owners, agents, officers, directors, employees and affiliates from and against any and all loss, liability, damage , cost, expense (including reasonable attorney’s fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whatsoever that I may sustain in connection with the services provided to me by EOS or any of the foregoing.
READ ME
To properly service you, we will contact you via email regarding your stay. You are required to submit your forms a minimum of seven days after receiving your booking confirmation with the exception of your flight information which may be submitted no later than seven days prior to your check in. You are also required to notify us once you are cleared. Failure to respond to email correspondence and/or forms will result in cancellation of your stay and you forfeit all monies paid.
