  • The midterm evaluation provides the student, supervisor and university with the opportunity to take note of the student's performance and progress.  it is recommended that both supervisor and intern take some time to discuss progress before signing for final submission.

    This evaluation form is designed for flexibility.  The student and supervisor will ideally sit down together at one computer to fill out and sign.  However, understanding constraints of time and availability the entire process may be completed electronically with each individual filling out his/her respective part(s).

    To be filled out by the student.
  • *The internship supervisor may be a point-person for the organization in a particular leadership role, but it may also be a qualified individual with whom the student works on a regular basis.
