This questionnaire is designed to help CT Pregnant Dog and Cat Rescue and yourself determine if a specific dog or cat is the right companion for you and your household. All adopting parties are required to complete this form prior to adoption.

Your application will not be processed if you do not fill in required information, including a personal reference and the name and phone number of your vet. Please do not expect our volunteers to search for phone numbers. Also, you must inform your vet that they have your permission to speak with CT Pregnant Dog and Cat Rescue.

CT Pregnant Dog and Cat Rescue reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!

Please use this same application if you wish to foster a cat or dog and note in the first line (name of cat or dog) that you are applying to FOSTER.

CT Pregnant Dog and Cat Rescue is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of stray, neglected and abused pregnant dogs and cats. CT Pregnant Dog and Cat Rescue is completely funded through fundraising efforts and public donations, including adoption fees.

All of our pets are cared for in foster homes and receive complete routine medical care, which includes deworming, flea/tick treatment if needed, age appropriate vaccinations, and spaying/neutering (for all dogs and cats over 6 months) prior to their adoption.

In addition to these expenses, we pay for transport to CT, for the food and supplies used by our fosters and for unexpected medical care such as treatment for heartworm, parvo or other serious and expensive health issues.

Please note that there is an adoption fee for all cats and dogs.

While we cannot make any guarantees as to the breed, personality or history prior to coming into our care, we do our best to try to match pet personality to families.
