CAU Bands Performance Request Form
Please fill out this form as completely and accurately as possible. All University/on-campus requests must be made 10 business days in advance, and must be sent to our department chair Mr. Phil Davis (pdavis@cau.edu). All metro Atlanta performance requests must be made a minimum of 15 business days in advance. All performance requests within the state of Georgia must be made a minimum of 30 days in advance. All out-of-state performance requests must be a minimum of 3 months in advance. All international performance requests must be made a calendar year in advance.

May 3 - 7, 2027 (Finals Week)
