If you must forfeit a game, you must complete this form to notify the league, your opponent, and our referee assignor. Teams must give a minimum of 48 hours notice to avoid paying the full referee fee for that match, which is $200 for full-sided games (your $100 plus your opponent's $100) and $60 (your $30 plus your opponent's $30) for 7v7 games. Complete this form whether you are giving the minimum notice or not.
