SEIU 775 Product Order Form

Most orders take 5-10 business days to process and will be shipped via UPS. (Individual contracts may be sent via the US Postal System.)

MATERIALS AVAILABLE:

New Employee Orientation (NEO) packets (Consumer Direct Washington and IPs only). NEO packets contain:
- One English membership form
- One full Collective Bargaining Agreement (CBA) with Consumer Direct WA
- One WA voter registration card

This will ensure we send you the right contract and ensure we pre-print the correct Employer on any Membership Forms you request. (For example, if you work for DSHS, are an IP or serve IPs, then pick the first item on the list, "Consumer Direct WA - IPs").

Each packet contains an English membership form, WA voter registration form, and full CBA for Consumer Direct WA.

8. How many copies of the full Collective Bargaining Agreement of the above selected employer do you need?

9. Membership Forms. (Please indicate the number of Membership Forms you need. If you are CDWA or an IP, these are in addition to the membership forms included in NEO packets, if you order those.)
