CANCELLATIONS (While we understand emergencies and life happens, repeated no shows will result in advance prepayment for future appointments and/or refusal to book in the future.)

- within 24 hours of transport date & rebook: 30% rebook fee
- within 24 hours of transport date & MEDICAL rebook: 10% rebook fee
- NO refund for complete cancellation within 24 hours of transport
My signature is verification that 1. I understand I am responsible for prompt transport fees and/or cancellation/rescheduling fees as outlined above. 2. In the case of a medical emergency, handlers/drivers will seek veterinary care for your pet at the expense of the Rescue. 3. I hereby consent and authorize No Stress Pet Express, LLC (hereafter noted as NSPE) to receive and transport my pet(s). I understand all reasonable precautions for the safekeeping of the above pet(s), but NSPE or any volunteers, representatives, contractors or employees will not be held responsible in any manner whatsoever on account of medical situations that may arise. I further understand the NSPE policy stating that all vaccinations must be up to date at the time of boarding transport (to include distemper and rabies for cats and distemper, rabies and bordetella for dogs. If my pet requires special food, it will be my responsibility to provide such food for the duration of the transport; otherwise, I understand that NSPE will supply dry kibble. I admit my pet is healthy and free of coughing, gagging, sneezing as well as nasal discharge associated with infection. I also know my pet has not been in contact with other pets who have shown above symptoms in the past two weeks. I understand NSPE takes utmost care in keeping their kennels disinfected and clean; therefore, NSPE reserves the right to refuse passengers exhibiting any of the symptoms above, and the refund policy will be effected. Pets infested with fleas and/or ticks at check in time will not be able to ride and no refund will be given. Initial balances for transportation must be paid in full to reserve a spot. A fee of $50 will be charged for any insufficient funds. *
