Fine Arts Graduate Students will be validated by the faculty. Your card will be programmed to access FB630. Fine Arts will cover the cost of a new card.
Please give details in the fields below. You will be charged $20 by security when you pickup a new card.
Fine Arts HUMA students will have the $20 security fee covered by Fine Arts.
HUMA students (non Fine Arts) will need to pay a $10 fee at EV8.465 prior to the form being processed.
