Before you get started:
1. Review the grant guidelines (PDF document) provided on the website prior to completing this form.
2. Complete the Word version application of the grant first, so you can prepare your answers ahead of time and proof the content. When your final draft copy is ready, set aside 20 minutes to complete the online application (you can simply copy and paste your answers from your word document). The online form must be done all at once. You will not be able to exit the form and save it.
3. Upon completion of this form you will receive a confirmation email with a copy of all of the questions and your answers. Please make sure to check your junk email folder. If you do not receive a confirmation email within 24 hours, please contact us.
4. Please read each question carefully. The number one reason proposals are declined is because there is a lack of detail and clarity in the application. Assume the review committee knows nothing about your organization or program! And develop a plan before applying!
