I, [full name], being of legal age, have requested to participate in the activities and travel associated with the Student International Business Council (“SIBC”) sponsored by the University of Notre Dame du Lac (“the University”), Notre Dame, Indiana. I understand and acknowledge that I am not required to participate in the activities and that my participation is wholly voluntary.
Furthermore, I elect to operate either a personal vehicle or a University vehicle to transport myself and other SIBC representatives to our intended destination. I understand that I have the sole responsibility to operate this vehicle and will not, without approval from SIBC or University officials, allow other individuals to operate the vehicle for the duration of the travel.
I understand that SIBC will reimburse me for highway tolls and reasonable parking fees, provided I present proof of purchase (e.g., receipt or photograph). I am responsible for any tickets, toll violations, or parking violations SIBC or the University receives associated with my vehicle throughout the duration of the travel.
As a student driver, I acknowledge that I have completed driver training with the University of Notre Dame Transportation Service and have read the organization’s student driving resources: Student Policy | Rental Guide
In signing this Waiver, I hereby acknowledge and represent that I have read this entire document, that I understand its terms and provisions, that by signing it I am giving up substantial legal rights, that it is a binding agreement, and that I have signed it knowingly and voluntarily.
