$300.00 Deposit (non-refundable) or Payment in Full must accompany your completed registration form. You will receive an email and/or text welcoming you into the program upon receipt of this enrollment form.
If you are just submitting your deposit at this time, we will set you up on a payment plan for the remaining tuition due. Payment plans are offered on a 6 month installment contract, and are set up on your first day of training.
Tuition balances must be paid in full by the final month of the current teacher training program you are enrolled in to be eligible to receive your certificate of completion.
Please complete this form and attach a one-page introduction of yourself, including any yoga experience, who you have studied with and for how long, teaching experience, if any...and any physical limitations. Also include what you hope to gain from your studies in this course.

You understand the inherent risks of practicing yoga and waive any claim against Total Harmony Yoga and its teachers, as well as for loss, expenses, liabilities, injuries, damages or legal fees incurred as a result of attendance at classes and trainings conducted by Total Harmony Yoga.
