The event dates selected will be confirmed via email/SMS or phone within 48 hours from submission of this form.

When you (hereafter referred to as "The Studio") complete this section of the form you are only requesting to engage the services of Chris Casamassa (hereafter referred to as The Artist) to attend and teach The official BULLY PROOF Event for the investment as outlined below.

The investment for the BULLY PROOF workshop is U.S. $2,500.00 and is only payable once an event date is secured. Investment fee for the Bully Proof clinic is due 7 days prior to event unless otherwise agreed.The Studio also guaratees to cover/reimburse up to 2 night(s) hotel accomodations, travel to and from airports (if required) and meals for The Artist for the duration of his visit.

The Studio will also reimburse the Artist for his airline flight to and from the event. The artist at his own expense may bring additional team members to help and suport with the event.

Should the event be cancelled or rescheduled by The Studio after confirmation of dates, The Studio will reimburse The Artist for airline tickets purchased. (Exception to rescheduling/cancellation would be any emergency, health or family issues and severe weather conditions where normal travel is affected.)

As a bonus, the Artist will provide a complete "publicity kit" for The Studio to use in any and all media, for publicicty and advertising promotion, Plus up to 50 copies of his Best selling Bully Proof book and up to 100 autographed photos. The Artist will do interviews with local media outlets as secured by the Studio. The Artist will stay beyond the scheduled event time to take pictures, do meet and greets and sign additional SWAG.

The studio agrees that anything "in addition to" the terms above is extra and may be negotiated by the artist and the studio.
