BY CLICKING ON THE SUBMIT BUTTON BELOW YOU ARE AGREEING TO THE FOLLOWING:
- Your instructor will be notified of this Request, asked to confirm all the details, and must provide a copy of the exam/test/quiz to DSPS OR online in your Canvas for the course.
- Extended time and all other approved accommodations for the test/exam/quiz MUST appear on the student's Academic Accommodation Plan (AAP) at the time of taking the exam. The Accommodations are the same ones listed on the Letter of Accommodation you sent to your instructor via the DSPS portal at the start of the course.
- You can view your AAP/LOA in your DSPS Portal online at anytime.
- Please schedule an appointment to meet with your DSPS Counselor to request additional accommodations if needed, well before the exam date.
- On the day of your Exam/Test/Quiz:
- Please check in at the DSPS office window at least 10-minutes before the start your exam; do not access the testing room without notifying DSPS staff of your arrival.
- You will be instructed to place all your personal belongings into a locker; staff will be on site to assist with the process if you need to use your cell phone to complete the 2-factor authentication process- then you will place your cell phone in the locker too.
The facilitation of tests by the Disabled Student Services staff at Columbia College is an extension of the academic course in which you are enrolled.
Typically, all policies and procedures that apply to tests given in the classroom will also
apply to tests administered by our department.
For example:
- Only pre-approved paper, notes, books, calculators, spell-checkers and other materials will be permitted in the testing room. (Lockers to secure your belongings are provided).
- Only pre-approved food or drink required for medical purposes will be permitted in the testing room.
- Only approved Accommodations breaks are allowed.
The test will be proctored only by DSPS personnel. DSPS staff may enter the testing room/area at any time while monitoring the test taking process. If any evidence of cheating is discovered, the exam will be terminated immediately and all materials will be confiscated. The course instructor and the Dean of Student Services will be notified immediately. Course penalties and disciplinary action will be determined in accordance with the policies and procedures stated in the College Catalog and the Code of Student Conduct.
We welcome all questions about DSPS Test Facilitation (also known as test proctoring). Students should make an appointment with their DSPS Counselor. Instructors of the course may direct their questions directly to the DSP Counselor or to the Test Facilitation staff as needed.
