BEHAVIOR POLICY: The parents and/or guardians promise to abide by all Grove School rules, including, but not limited to, those set forth in The Grove School Student and Family Handbook 2018-2019 (see link below), and those set forth by The Grove School’s Head of School and Board of Trustees for the 2018-2019 school year. MEDICAL RELEASE, FIELD TRIP CONSENT AND LIABILITY WAIVER: I hereby give permission for my student to participate in Armantrout Montessori Education Foundation daytime field trips during either or both sessions of Summer in the Grove 2019. I understand field trips may require transportation to a location away from the school campus. As a parent or guardian, I understand that the Foundation and its staff will try to prevent accidents. However, I fully understand that some activities on field trips involve inherent risks to students regardless of all feasible safety measures that may be taken by the Foundation. In consideration of the Foundation's agreement to allow my child to participate in field trips, I agree to accept responsibility for any loss, damage or injury to my child that occurs during my child’s participation in this field trip that is not the result of fraud, willful injury to a person or property, or the willful or negligent violation of the law by a trustee, employee, or agent of AMEF. In the event it becomes necessary for school staff to obtain emergency care for my child, neither the staff nor AMEF assumes financial liability for expenses incurred. I authorize AMEF employees or volunteers in charge of the students to obtain all necessary emergency medical care and authorize any licensed physician and/or medical personnel to render necessary emergency treatment for my child.
RELEASE AND HOLD HARMLESS AUTHORIZATION FOR A MINOR: My child’s participation in activities at AMEF’s Summer in the Grove is with the understanding and acceptance that there are risks of harm or personal injury in any activity involving children; we hereby release, acquit, discharge, and hold harmless AMEF against any liability resulting from any injury that may occur to my child while participating in AMEF-related activities.
JURISDICTION/VENUE/LEGAL FEES: This Contract is to be construed under the laws of the State of California. The parties agree to the jurisdiction and venue of the State of California, County of San Bernardino. Should action be brought to enforce or interpret the provision of the Contract, the prevailing party shall be entitled to legal fees in addition to whatever other relief is granted. The person(s) executing this Contract on behalf of the student are hereby representing that they have the legal authority to do so. If there are any court documents or powers of attorney that give this/these person(s) the authority to do so, a certified copy will hereby be supplied to AMEF and included in the student’s file.
PHOTO RELEASE: The Armantrout Montessori Education Foundation has my permission use to my child’s photograph publicly to promote the Foundation and its activities, including Summer in the Grove. I do this with full knowledge and consent, and waive all claims for compensation for use, or for damages. I understand the images may be used for any lawful purpose, including publicity, illustration, advertising, and Web content. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use. If I do not authorize AMEF to photograph my child, I understand I must communicate this VIA EMAIL to the AMEF Administrator at email@example.com, and agree that my non-consent is effective upon receiving acknowledgment of receipt of the aforementioned emailed non-consent from the Summer in the Grove Administrator.
REFUND AND CANCELLATION POLICY: Summer program enrollment is on a first-come, first-served basis. Full payment upon registration is strongly preferred; however, a $100 deposit will secure your student’s place in the summer program. Summer program deposits are non-refundable and can only be applied toward the current year summer program fees. The last day to cancel, make changes and pay outstanding tuition is May 15, 2019. New enrollments may or may not be accepted after that date at the discretion of the AMEF Administrator. All deposits and fees will be forfeited if a cancellation is made after the deadline. A limited number of scholarships are available to students who qualify for free or reduced school lunch. To apply for a scholarship, email firstname.lastname@example.org.
