ALL MASS INTENTION REQUESTS ARE SUBJECT TO APPROVAL. FILLING OUT THE REQUEST FORM DOES NOT NECESSARILY MEAN THIS DATE/TIME WILL BE YOURS UNTIL IT IS REVIEWED AND APPROVED BY SAINT JOHN'S OFFICE PERSONNEL.
