NO REFUNDS OR CREDITS SHALL BE ALLOWED UNLESS ALL PAYMENTS OR ACCOUNTS ARE CURRENT.

INVOLUNTARY WITHDRAWAL (illness, job transfer, death, or expulsion):
Pro-rated refund based on time remaining in the school year. There is a 100.00 processing fee for all early withdrawals.

I (we) agree to see that the Academy's standards for appearance, dress, and conduct will be complied with. I (we) agree with the Academy's policy on discipline. I (we) also understand that pre-registration and acceptance of the student by the Academy shall be for a full year's term; and the student and his parents or guardians shall be liable for full year's tuition, regardless of the method chosen for payments, the only exception being involuntary withdrawal or voluntary withdrawal within the first five weeks. I (we) agree to supply a copy of the student's birth certificate (for all students entering Junior Kindergarten through High School.) All students should be able to speak English. I (we) have read and understood the school's statement of faith and philosophy.

Books received in 1st-12th are the property of CLA, and will be returned in the condition received. I agree to pay for any lost or damaged books.
