We would like to help you track down the dead end microchip (or tag, or address, or owner name or phone number) associated with a found animal. Please include as much information as you can in the following fields:

Microchip Number/Rabies Tag/ID Tag- Please note a valid microchip number can only be 9, 10, or 15 characters. Some microchips do have letters, but if the length is not 9, 10, or 15, have the chip rescanned, because the number is incorrect. EXCEPTION- If the chip number starts with TR, or if the animal also has an ID or Rabies Tag, put information in the Comments field at the end or send a photo. (Put 000 in this field if no microchip.) *

Animal ID for owner reclaim - This allows us to pull up the animal's photo on your public website to check on the status. We stop our work if an animal is no longer available. If not applicable, say NONE*

10 digit alpha-numeric entry

If this animal will not be on your website, please check the box below and provide a photo OR a description of the animal in the box for additional information. We monitor the animal's listing to ensure the animal is still available, checking this box means we will not follow up to ask if there is an error.

We ONLY say the animal is under veterinary care, even if we know the reason, we say the only info we have is under veterinary care.

Do you have adoption records for this animal or contact information from reuniting the animal previously? Did you speak to a rescue that implanted the chip? Do you check utility records on microchips? Please let us know what you know. It does not matter if the phone number is disconnected, we can still use it to try and find the owner.
