Great! Let's begin the Finish the Mission Relief Fund grant application process.
In addition to completing the following online form you will need to be prepared to provide the following information:
- Written proof of Service and Honorable Discharge from the United States Armed Services (Discharge papers, separation report or form DD-214)
- Supply proof of residency in Kent Count Michigan (utility bill, rental statement, mortgage, proof of auto insurance, etc...)
- Your recent application for Emergency Relief from Kent County Veterans Services
- A copy of your Emergency Relief denial letter from Kent County Veterans Services
If you have electronic versions of these documents available, you will be asked to upload them at the end of our online form.
If you do not have electronic versions of these documents available to upload you will be directed on how to provide them to accompany this online grant application form.
