Friday, September 20 - All Skills - FULL - REGISTRATION CLOSED
Friday, October 4 - Hitting -Blocking - Setting - FULL REGISTRATION CLOSED
Friday, October 18 - Serving - Defense -FULL REGISTRATION CLOSED

Time: 6:15-8:45 pm

Cost: $30 per session OR $75 for all 3 sessions (save $15)

Parent/Guardian Consent

I hereby state that my child is in good health, and has my permission to participate in all activities. In addition, I authorize MBS Training Club’s staff to act for me in securing medical treatment for my child in the event of injury or illness. A registration requires that a parent or guardian sign below to agree that in case of an accident involving my child while attending the program, they release the program, sponsors, teachers, coaches, or any other staff of MBS Training Club from any and all liability. I also consent to the collection and use of my child’s personal images by photography or video recording. I acknowledge these may be used on the company’s website, in newsletters and publications as well as distributed to members. I also understand that my consent can be withdrawn at any time by submitting a written request. I have read the release and this agreement carefully, fully understand their contents, and voluntarily agree to their terms.
