We recognize that throughout a term, an officer may be removed from a State Association's team for a variety of reasons. This form is distributed to the necessary parties at National FFA so their contact information is removed from various databases organization wide.
Upon receipt of this form, the officer will be removed from the following places:
- State Officer Database on FFA.org
- State FFA Officer Facebook Group and Instagram Account
- State Officer Academy
- Growth Index Reporting Tools
- Distribution Lists used by FFA's Marketing Team
NOTE: This form is NOT for retired state officers. We keep them in our databases for various reasons. Please only use this form for officers who have either resigned or been removed from office during their year of service.
