The agreement dated the date set forth below, by and between Mike's Chance Animal Rescue, a 501C3 Corporation and the individual whose name is set forth on the signature page of this agreement (Adopter)

**The breed of the rescue animals determined to the best of our knowlage about this animal and based on images and a description recognition by an artificial intel and not guaranteed.

Preliminary Statements

 The Adopter desires to become the owner of the animal and the Mike's Chance Animal Rescue is willing to transfer ownership to the adopter, provided Adopter strictly agrees and strictly complies with the terms of this agreement.

 Ownership: The Mike's Chance Animal Rescue hereby transfers ownership of this animal to the Adopter , and Adopter hereby accepts ownership of the animal, subject to all terms and conditions of this agreement.

Adopter's Responsibility:  In consideration for transfer of ownership of the animal to the Adopter, the Adopter agrees to the following:

 a.  Provide good care, shelter and companionship to the animal and abide by all law and ordinances of the community in which the animal will reside.

b.  Take reasonable precautions against theft or loss of the animal.  If either of the forgoing occur, Adopter agrees to notify local pounds, shelters and police immediately, to advertise for the return of the animal on pet recovery Lost & Found sites like (pawboost.com). 

 c.   To obtain veterinary attention whenever the animal is sick or injured and have a veterinarian administer immunizations as needed.       

 e.  To allow a representative of the rescue to call at Adopter's home to see the animal at any time and to RECLAIM the animal if it is determined the animal is not being properly cared for.

f.  To notify the rescue of any change in address or telephone number.

g.  To be responsible for the animal and its actions.

h. If an animal is not fixed due to the young age or any medical circumstances - To have the animal spayed/neutered within a reasonable times, when mature enough 8-10 months old, provide all the spay/neuter documentation to the Mikes chance animal rescue (by email or fax) and under no circumstances allow the animal to breed or bear a litter. An adopter or foster must stay in touch with a rescue until the animal is spayed or neutered.If at any time Adopter can not or does not wish to keep the animal, Adopter will IMMEDIATELY contact the Mike's chance animal rescue to inform and ask for help to place the animal in a new home and give a reasonable time to place it (2 months).  Notification MUST be made to the Mike's Chance Animal rescue by telephone, mail or email with verification of receipt in advance.  Adopter will NOT under any circumstances sell or give the animal away without FIRST consulting the Mike's Chance Animal rescue.                        

C.  Rights of the Mike's Chance Animal Rescue:  In the event the Adopter violates this agreement, the Mike's Chance Animal rescue shall have the right to take actions and ask for reasonable  compensation for the cost of expenses to recover the dog, treat it and other needed actions to save a dog. 

D.  Limitation on Liability:   Indemnification.  The Adopter acknowledges and agrees that Mike's Chance Animal rescue shall not be liable for any  defect that now exists or herein may arise with the animal.  The Adopter further releases  Mike's Chance   from  any liability arising out of the actions or conduct of the animal after the date hereof  including injuries to persons and animals and damage to property.

E.  Donation-adoption fee for dog adopters: An adopter will pay an adoption fee that includes transportation fees and  other pet expances. It can be split in two payment- AS IT IS SPECIFIED IN THE ADOPTION EMAIL that was sent to the adopter.  Total payment should be recieved on the day of the adoption or prior to it.

*We leave rights to reclame the dog or recover none payment through a collection egency in case of not recieving the adoption fees in full.

F. For cats adopters: An adopter will commit to pay a minimum of $100-350.00$ mandatory fee to the Rescue at the time of adoption that include any transportation fees, as specified.

*  Transportation, transaction or any other fees are not refundable after transport was booked by the rescue or upon the animals delivery.
