      -- One person per application. 

      -- Please submit a headshot for the WPPA directory (2x3 jpeg or tiff at 300 dpi)

      -- Application must be signed and dated, with full payment remitted. Remit by clicking on payment link under Membership Payment Tab.

      By completing and submitting this document attests that all statements are true to the best of my knowledge and further attests that I have read and agree, without reservation. 

      I will recognize the authority of the WPPA and agree to hold harmless the Board of Directors in any decision made on behalf of the corporation.

      All applicants are examined by and are subject to approval by the Board of Directors. Applicants will be notified of their application status by a member of the WPPA Board of Directors.
