Dear Faith Formation Family,
Accurate and complete information is essential to ensure that your registration is processed promptly.
A completed registration form for returning families must be received before July 31st or a late fee of $30.00 will automatically be added to your tuition.
The registration process is a two-step process:
Step 1: Completing this form in its entirety.
Step 2: Updating your FACTS tuition management account
Both steps above MUST be completed in order for your child(s) to be fully registered and receive a registration confirmation email.
The Faith Formation program utilizes a tuition management company to process tuition payments. All families must have a FACTS account created even if paying by check. If paying by check the full amount must be received.
August 31st is the last day to register in order for your child to have a book on the first day of class.
