Please provide copies of the following documents to us. These can be mailed to the office, dropped by, or submitted electronically using a secure upload link. If you would like to upload the documents electronically, please reach out to an advisor so they can create an upload link for you. If you decide to mail or drop by with original documents, we will image them and return them to you.
- Tax returns, last 2 years federal, 1 year state. Include schedules and 1099’s, etc.
- Last Social Security Benefits Estimate Report (The Social Security Administration rarely sends out written reports anymore. You can retrieve your full Social Security statement online at www.ssa.gov/myaccount You will need to create an account and request the full statement, which can be downloaded and printed.) We need the full statement, with the full wage history being key.
- A recent paystub for all employed persons excluding children
- If covered by an employee pension, the most recent pension benefits estimate statement (ex. State Retirement)
- Most recent statements for all investments, 401(k) plans, stocks, bonds, CD’s, etc. You do not need to provide statements for investments held through our firm. If a holder of CD’s, information about interest rates and maturities of each CD.
- Cost basis information for non-retirement investments.
- Information regarding life insurance coverage, including employer coverage
- Any information on debts, mortgages, etc. including original balance, current balance, payment, interest rate, terms, etc.
- A copy of your will, powers of attorney, or trust document
- A list of beneficiary designations for insurance and retirement accounts
