    Application for Admission to Middle and Upper School Grades 6-12
  • Apply to Grades 6-12 for 2025-26 or 2026-27

    Please use this form to apply for admission to Grades 6-12. There are two questions the student shoud complete, and you can either include answers in this form or send to admissions@cislions.org. After you submit the application, the CIS Admissions office will contact you regarding next steps, including additional forms, the entrance exam, and a shadow visit/interview.

    For current year enrollment in 2025-26, we will accept applications until January 9, 2026, for enrollment no later than January 31, 2026.

    For enrollment in 2026-27, the initial application deadline is March 20, 2026. We will continue to accept applications after this date until grade levels are full.

    Note regarding payment of the $70 application fee in this form: our payment processor Authorize.net accepts Visa, Mastercard and Discover - sorry, they do not accept American Express.

     

    CIS believes a strong, cooperative partnership with families is a key component of student success, and this partnership begins at the applicant stage. If more than one parent or guardian has physical and/or legal custody of the student, both parents or guardians should be listed in this application and will be asked to sign an enrollment agreement before the student can be enrolled. This is our policy even when parents are divorced or if only one parent plans to pay tuition.
