I have read the vaccine policy and will email or fax any vaccines shown on my confirmation email.

I will provide yearly stool sample results and understand that P.A.W.S goes above and beyond to keep their guests healthy, safe and parasite free.

I understand that my dog is involved in supervised social group play and that this exposes my dog to the possibility of picking up a cold or virus or possible parasites from the other dogs.

I understand that due to group play my dog may lose weight when boarding due to all the activity and I will provide a possible increase in daily food to compensate for the activity.

I understand that the dogs in the care of P.A.W.S. are fed TWICE a day for adult dogs 8 months and older, 3 times a day for dogs 7 months old or less, and once a day if you request only once a day.

If I have not heard back within 48 hours I will call P.A.W.S. immediately to confirm my reservation.

I understand that I will be charged for a NO SHOW reservation and have reviewed my confirmation emails for accuracy.

PAWS requests that I avoid dog parks due to the ease of contracting Kennel cough, respiratory and other illness, and internal parasites then bringing these into the PAWS environment unknowingly. As well as the possible unfavorable canine social behaviors that frequently develop. Thank you.

Cancellation Policy

For Holiday’s and School Vacation Weeks/PEAK VACATION PERIODS-WEEKS, scheduled at PAWS kennel/Doggie Hotel, Cancelations and Shortened Changes MUST be made 10 (Ten) days in advance. Fifty Percent (50%) of the total stay will be charged for late cancellations and you are responsible for "shortened reservations" during these peak times. Non-Holiday/Non-Peak reservations require 48 hours’ notice.

Daycare No Shows are charged as a Full Daycare Day, or If you have a Daycare Package a day will be removed from the package

Daycare Cancellations Require 24-48 hours’ notice to avoid being Charge for the Daycare Day. If you have a Daycare Package, a day will be removed from the package

Inclement weather and storms do not void the cancellation policy.

Unpaid balances are subject to both a processing fee and a billing fee at each billing cycle. A fee is applied to returned checks.

I have read, understood, and accept these terms including the cancellation policy. I have not altered this contract.

Contract Waiver, Assumption of Risk and Agreement to Indemnify and Hold Harmless

I/We understand that attendance is not without risk to me, members of my family or guests who may be on The Professional Animal Wellness Services, LLC (P.A.W.S.), premises, or to my dog, because some of the dogs to which I(we) may be exposed may be difficult to control and may cause injury, even when temperament tested, supervised and handled with the greatest amount of care. I hereby waive and release Professional Animal Wellness Services, LLC (P.A.W.S.), its employees, owners and agents from any and all liability of any nature, for injury or damage which I or my dog may suffer, including, but not limited to any injury or damage resulting from the action of any dog(s); and I expressly assume the risk of any such damage or injury while on the premises of Professional Animal Wellness Services, LLC (P.A.W.S.), or in the surrounding area. In consideration of and as an inducement to the acceptance of my application for participation, I/we hereby agree to indemnify and hold harmless Professional Animal Wellness Services, LLC (P.A.W.S.), its employees, owners and agents from any and all claims, or claims by any member of my family, guest, or any person accompanying my dog to Professional Animal Wellness Services, LLC (P.A.W.S.), premises or in the surrounding area as a result of any action by any dog, including my own.

I, the undersigned, hereby authorize Professional Animal Wellness Services, LLC (P.A.W.S.) and its representatives to remove mats, tangles, and excess fur from my dog, {petsName} if deemed necessary. I understand that while every precaution will be taken to ensure my dog’s safety and comfort, there may be inherent risks involved in the mat removal process, including but not limited to potential skin irritation, nicks, or cuts.

I confirm that my dog has not exhibited aggressive behavior that could pose a risk to the groomer or others during the process. I agree to release Professional Animal Wellness Services, LLC (P.A.W.S.), its employees, and agents from any liability arising from the mat removal service, except in the case of gross negligence or intentional harm.

I also confirm that my dog is up-to-date on vaccinations, including rabies, and has no known medical conditions that may interfere with the grooming process.

I have read, understood, and accept these terms including the cancellation policy. I have not altered this contract.
