NO REFUNDS
AGREEMENT TO HOLD HARMLESS, WAIVER AND ASSUMPTION OF RISK
I understand that attendance of a dog obedience training class is not without risk to myself, members of my family or guests who may attend, or my dog(s), because the dog(s) may be difficult to control and may be the cause of injury even when handled with the greatest amount of care. I do hereby affirm that I have disclosed ANY AND ALL previous or current aggression that my dog(s) has displayed towards humans or other animals. I also affirm that my dog(s) will be kept on a leash and collar for the entire time that myself and my dog(s) are within the boundaries of the property where the training is being held, unless otherwise directed by the trainer. I do hereby affirm that prior to the start of the training session, I will have my dog(s) in the proper leash, collar or other equipment that the Trainer has instructed me to use.
I understand that causing a distraction including specifically, but without limitation, noise, personal presence, food, introduction of other animals or humans, or any and all other distractions deemed by the Trainer, can cause danger and/or injury to the Trainer, my dog(s), and other humans/animals present. I do hereby affirm that I will not cause any type of distraction from the start to the end of each training session. I do hereby affirm that I will follow any and all instructions given by the Trainer.
I hereby waive and release Texas Coast K9 Training LLC, Texascoastk9.com hereinafter referred to as the "Training Organization and Affiliates", its employees, officers, members and agents from any and all liability of any nature, for injury or damage which I or my dog(s) may suffer, including specifically, but without limitation, any injury or damage resulting from the action of any dog(s), and I expressly assume the risk of such damage or injury while attending any training session, or any other function, of the "Training Organization and Affiliates", or while on the training grounds, my property or the surrounding area thereto.
In consideration of and as inducement to the acceptance of my application for training by the "Training Organization and Affiliates", I hereby agree to indemnify and hold harmless, the "Training Organization and Affiliates", its employees, officers, members and agents from any and all claims, or claims by any member of any family or any other person accompanying me to any training session or function conducted by the Training Organization, or while on the grounds, my property or the surrounding area thereto, as a result of any action by myself or any dog(s), including my own.
I do hereby agree to full payment of quoted price by the "Training Organization and Affiliates", which to include all training/behavior correction(s) originally agreed upon. I do hereby attest to my understand that any and all training requested by me, my family, or any of my affiliates that is outside of the original training/behavior correction(s) agreed upon, will be at an extra cost.
I also do hereby agree to either FULL payment on the FIRST session of training, or 50% of the quoted price at the START of the FIRST session and 50% of the quoted price at the START of the THIRD session of training. No other payment plans will be allowed unless otherwise specified in writing by the "Training Organization and Affiliates". I attest that I understand that NO further training sessions will begin unless this payment agreement is met in the structure noted above.
I do hereby agree that any cancellation or postponement of a session by me, my family, or my affiliates, without 24 hours’ notice prior to the session start time, will result in a forfeit of this session. I understand that this session can be purchased back for the amount of $50.00, which will be required to be paid in full at the start of the next session. I attest that a forfeited session may result in my dog(s) not receiving the full training and/or behavior correction(s) that was originally agreed upon.
