Requirements: *Renters and HOA: If you rent or live in a condominium or co-op or your house is part of a homeowner’s association, you must provide a copy of the bylaws, rules and regulations of that association stating that there are no restrictions regarding owning an animal. If there is a limit on the number of pets you are allowed, please note that limit on the copy of the bylaws. Cubby’s Crusade fully vets all rescue animals. It is at the discretion of Cubby’s Crusade of which vets are acceptable. All information, vet visits, etc. are to be listed under Cubby’s Crusade as the owner of the foster animal. Any vet visit must have approval by the rescue. In the case of an emergency, I agree to contact the rescue immediately. I understand I am responsible for getting all documentation (shelter paperwork, medical records, vet visits, etc.) sent to director in a timely manner. I agree to keep director updated on the well-being of my foster animal. I agree to send presentable photos and updated information of the foster animal to the director. I agree to allow Cubby’s Crusade or a representative to periodically visit my home. I shall, upon request, immediately release any foster animal to a representative of the organization within 24 hours of the request by a board member. I understand that I am not the legal owner of the animal in my care and if I choose to adopt, I must contact Cubby’s Crusade to discuss this possibility and follow the adoption process and that is the discretion of Cubby’s Crusade for approval. I agree that the animal will wear an identification tag with current phone number. I agree to provide a safe foster home to include: indoor shelter, adequate food/water, exercise, love and affection, and to contact Cubby’s Crusade immediately should the foster animal become ill, injured or lost. I agree to represent Cubby’s Crusade in an honorable manner and will act accordingly in my dealings with the public at any Cubby’s Crusade events. I understand that Cubby’s Crusade has limited knowledge of the animals’ health and temperament coming into the rescue from shelters or owner surrenders. I understand and agree that I will not hold Cubby’s Crusade responsible for any injury, illness or disease contracted by any animals privately owned, or for property damage, or harm to other animals or persons who may come into contact with the foster while he/she is in my care. I understand that I have no authority to place or promise any foster animal in my care to any home without written agreement from a Cubby’s Crusade board member. This includes returning the animal to his/her previous owner. In the event of abuse or injury to any foster animal or in the case of breach of contract, the foster animal must be returned immediately, with all animal related items provided with the animal, and all paperwork included. **Cubby’s Crusade requires a home visit. There are no exceptions.**
