Adopter: I do hereby adopt the above-named animal from Cubby’s Crusade. I understand and agree to the rules of Cubby’s Crusade listed in this contract. I agree that I have never been convicted of animal cruelty, neglect or abandonment and have no such charges pending. I agree that I am at least 18 years of age. ALL adults in the household must agree to the adoption. I agree that I am adopting this animal for myself and he/she will not be sold, adopted or given to another person. I agree to keep Cubby’s Crusade informed of my current home address and phone number. I agree to contact Cubby’s Crusade if the animal is lost, stolen or deceased.
Adoption Donation: To assist us in our rescue work, we require an adoption donation. Your non-refundable adoption donation only covers medical care given to date.
Veterinary Care: You agree to have the animal examined by a licensed veterinarian not less than yearly. In case of illness or injury, you agree to seek prompt veterinary care. You agree to keep all vaccinations current, including but not limited to Rabies and Distemper/Parvo and to ensure the animal receives preventative medication to protect against heart worm. You consent to Cubby’s Crusade inquiring with your veterinarian about the animal after adoption and to us looking over your property at a mutually convenient time. By signing this contract, you agree that you are willing and able to provide annual costs on medical care and food for the animal throughout his/her lifetime.
Behavior: If the animal has a known temperament problem which restricts adoption, this has been discussed (no kids, animals, etc.). I agree to follow any and all extra guidelines.
Care of the Animal: You agree to provide the animal with fresh water, wholesome food, adequate outdoor exercise, affection, vaccinations, medical care and shelter. You will treat the animal as a household pet, companion and family member, receiving proper nourishment, medical care and attention. You agree to not keep the animal as an outdoor pet. Continuous outdoor confinement is not acceptable. Doghouses, pens, chaining and runs are unacceptable. Animals should never be left unattended in the yard. You agree that this animal will live inside your home, not outdoors.
Identification: You agree that the animal will wear an identification tag with current phone number. Cubby’s Crusade provides microchips to all adopted animals to ensure further safety.
Problems: If at any time there are any problems with the animal, I will contact Cubby’s Crusade and seek their council for a resolution.
Transfer: If at any time I need to relinquish ownership of the animal for any reason, I will contact Cubby’s Crusade and return the animal at a designated time. I understand that if the animal is returned to Cubby’s Crusade adoption fees and other expenses incurred will not be refunded to me. The animal MUST be returned to Cubby’s Crusade at my own expense if the adopter can no longer care for the animal properly. Failure to return the animal will result in litigation. I agree that I am not to send the animal back by air and that the animal will be driven back to Cubby’s Crusade either by myself or at my expense. If a Cubby’s Crusade volunteer travels to retrieve the animal, I will be financially liable for the gas and toll expenses they incur retrieving the animal. I agree to the following additional expenses if the volunteer must travel farther than a total of 500 miles round trip to retrieve the animal: $25 per day for meals (no receipts required) and 1 day’s lodging for every 500 miles driven to retrieve the animal (lodging reimbursement limited to $100 per day). All expenses will be documented. The venue for any litigation related to enforcement of this contract shall be Charlottesville, Virginia. I agree to be liable for Cubby’s Crusade’s legal fees if they sue me for enforcement or breach of this contract.
Home Visit: I agree to allow an adoption representative to visit my home with or without prior notice to examine the animal or make inquiries as to how well the obligations of this contract are being met. If these obligations are not being met in the opinion of the adoption representative, a report will be made to Cubby’s Crusade board of directors to decide the appropriate action to be taken, up to and including reclaiming the animal. I agree to abide by the decision of the board and if it is necessary I will relinquish ownership of the animal to the adoption representative. Any evidence of abuse will be reported to proper authorities.
Liability: Cubby’s Crusade takes all precautions to ensure the animal you are adopting is healthy. While precautions are taken, Cubby’s Crusade cannot guarantee the health, behavior or temperament of any animal leaving the facility. Some animals may take longer to adjust to a new environment. Even a house-trained animal may need extra training as he/she adjusts to a new home. Cubby’s Crusade has limited information about the history of any animal we adopt out, and must rely on the accuracy of information provided by any person surrendering the animal, shelter, evaluations made by veterinarians or foster. For these reasons, the information you have been provided is not guaranteed. Any animal may bite or destroy property. Cubby’s Crusade staff and board of directors shall not be held responsible for any damages, injury or harm, costs for expenses incurred by the adopter resulting from this placement including but not limited to damage or injury to persons or property. The adopter releases Cubby’s Crusade staff, agents, representatives and board of directors harmless from any and all liability of any and every nature and cause, directly or indirectly relating to the placement of any animal.
I agree I have read and understand all of the above requirements and will follow them as written. This is a legal and binding contract and if for any reason legal action is necessary because of breach of contract, I will be responsible for any legal fees incurred.
