    Please complete this web-based submission form to request an enhanced Physician Directory profile. The request will fall into the queue for all website updates.

    Only physicians that are in the Physician Directory can have a corresponding profile and image. Here is the criteria for being featured in the Directory, as determined by Medical Affairs:

    1. The clinicians needs to be eligible for Reappointment (Permanent staff)
    2. The clinician needs to hold one of the following appointments (ranks):
      1. Active
      2. Active Associate
      3. Chief
      4. Active Provisional
      5. Active Associate Provisional
    3. The clinician needs a "Work" or "Practice" address in their record for one the following Sinai sites:
      1. 600 University
      2. 522 University
      3. 700 University
      4. 60 Murray
      5. 1 Bridgepoint Drive
  • Please provide your bio as a Word document or submit it below. Please include no more than 300 words. Your bio may be lightly edited to align with the Sinai Health Writing Style Guide.

    If you have an existing image, please upload it. The minimum size for the image is 800 x 1200 pixels. Please ensure there is space above and beside the head, as the image will need to adjust for different screen sizes.

    Should you need support with the image, please submit a request to Creative Services.
