Level Up Hoops Academy Release & Waiver of Liability & Indemnity Agreement* This field is required.**LEVEL UP HOOPS ACADEMY RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNIFICATION AGREEMENT** In consideration of being permitted to participate in any programs, activities, training sessions, clinics, practices, games, conditioning, camps, events, or other activities (collectively, the “Activities”) planned, organized, directed, or conducted by Level Up Hoops Academy, including but not limited to the use of any facilities, equipment, or services, whether in person or online, and under the direction of Andy Saharan or any staff member, the undersigned participant (or parent/legal guardian if the participant is a minor) agrees as follows: 1. Assumption of Risk I understand and acknowledge that participation in athletic activities, including basketball training and related conditioning, involves inherent risks, including but not limited to physical injury, illness, permanent disability, paralysis, or death, as well as damage or loss of personal property. I knowingly and voluntarily assume full responsibility for all such risks, both known and unknown, even if arising from the negligence of the Released Parties (as defined below), to the fullest extent permitted by law. 2. Release and Waiver of Liability I hereby release, waive, discharge, and covenant not to sue Level Up Hoops Academy, Andy Saharan, and their respective owners, directors, officers, employees, coaches, staff members, volunteers, board members, affiliates, representatives, agents, and contractors (collectively referred to as the “Released Parties”) from any and all claims, demands, causes of action, damages, losses, costs, or liabilities of any kind arising out of or related to my participation in the Activities, including but not limited to personal injury, illness, death, or property damage, whether caused by negligence or otherwise, to the maximum extent permitted by law. 3. Indemnification I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, damages, losses, liabilities, costs, or expenses (including reasonable attorneys’ fees) arising out of or related to my participation in the Activities or my failure to comply with any rules, policies, or instructions of Level Up Hoops Academy. 4. Medical Acknowledgment I certify that I am physically fit and have no medical conditions that would prevent safe participation in the Activities. I understand that Level Up Hoops Academy does not provide medical insurance coverage for participants. In the event of injury or illness, I authorize Level Up Hoops Academy staff to obtain emergency medical treatment if deemed necessary, and I accept full financial responsibility for any such treatment. 5. Compliance with Rules and Safety Policies I agree to follow all rules, policies, procedures, and safety guidelines established by Level Up Hoops Academy, including those intended to reduce the risk of injury or illness, including communicable diseases. 6. Acknowledgment of Legal Rights I understand that by signing this document, I am waiving substantial legal rights that I or my heirs, assigns, or personal representatives may otherwise have against Andy Saharan and Level Up Hoops Academy. I acknowledge that this agreement is legally binding and enforceable. 7. Voluntary Execution I affirm that I have carefully read this Release and Waiver of Liability, fully understand its contents, and sign it freely and voluntarily without any inducement or coercion.
