Membership Cancel Request

Please review your Membership Agreement and the Cancellation procedures form you filled out at signup, a copy was emailed to you.

Term Contracts may request cancellation once the original term (6 or 12 month) is complete.

Special circumstance cancellation may be requested if either of the following apply:

1) Medical restrictions preventing any future martial arts training

2) Relocation more than 25 miles from registered home address

Special circumstance cancellation requires the payment of a cancellation fee calculated as follows:  (current month-to-month price multiplied by number of months enrolled) minus dues already paid.

REQUIRED: Please use "Supporting Documents" button below to upload a Medical doctors note or proof of new address (Drivers License, Lease, etc).
