  • Please review your Membership Agreement and the Cancellation procedures form you filled out at signup, a copy was emailed to you.

  • Term Contracts may request cancellation once the original term (6 or 12 month) is complete.

    Special circumstance cancellation may be requested if either of the following apply:

    1) Medical restrictions preventing any future martial arts training

    2) Relocation more than 25 miles from registered home address

    Special circumstance cancellation requires the payment of a cancellation fee calculated as follows:  (current month-to-month price multiplied by number of months enrolled) minus dues already paid.

    REQUIRED: Please use "Supporting Documents" button below to upload a Medical doctors note or proof of new address (Drivers License, Lease, etc).
