Recertification

If you meet all recertification criteria (all points of this form are completed), your recertification is good for two years and you will receive a new certificate by email.
If your certification is late by one month or less there is a $50 late fee. If your certification is late by more than one month there is a $100 late fee/year in addition to the recertification fee and requirements. You can also choose to return to training at a 50% discount to meet all recertificaion requirements and fees, in addition to the recertification fee.

Allow 30 days for your recertification to be processed. If you would like to receive your certificate within seven business days, you can choose the rush option at checkout.

If you do not meet all recertification requirements, you will not qualify for recertification and will not receive a new certificate. If you do not meet all requirements you can return to training at a 50% discount to meet all recertification requirements. Please note, if you would like this added to your certificate to work with the OHCA, you must meet additional requirements and complete this required form. If you do not live or work in Oklahoma, this does not apply to you. If you serve Oklahoma, but do not wish to work with OHCA to provide services for Medicaid reimbursement, then you do not need this stamp and do not need to complete this additional form.
