Forfeit Submission Form - Men 7v7 (Over 35)
If you must forfeit a game, you must complete this form to notify the league, your opponent, and our referee assignor. Teams must give a minimum notice of 9:30 am the day before the match (30-36 hours, depending on kickoff time) to avoid paying the full referee fee for that match, which is $220 for full-sided games (your $110 plus your opponent's $110) and $70 (your $35 plus your opponent's $35) for 7v7 games. If you do not give at least 60 minutes notice on the day of the match, or do not show up with at least seven players by kickoff (five for 7v7), an extra $80 (total of $300) is added for 11v11 games, an extra $25 for 7v7 games (total of $95). Complete this form whether your are giving the minimum notice or not.
