- 1. Invoices will be sent to the email addressed specified for billing and must be paid by the deadlines specified to guarantee your space.
  2. Deposits and fees are non-refundable.
  3. In the unlikely event that Surrey Performing Arts has to cancel a session, a refund will be offered.
  4. Surrey Performing Arts reserves the right to withdraw a place for your child in the event of violent or disruptive behaviour (non-refundable).
  5. Children must wear appropriate clothing for dance and movement that they can be upside down in comfortably (i.e. leotard and leggings). A pupil may have to sit out of a session if dressed inappropriately.
  6. Children must be sent in with an appropriate lunch, snack and a filled water bottle.
  7. Long hair must be tied up upon arrival.

  Please read and tick the following in order to register. Please note that only the photo and email consent is optional as the other permissions are necessary in order to conduct our sessions.

- I understand that teaching dance may require appropriate physical contact with my child and give permission for a teacher to do so in accordance with safe dance teacher practice
- I consent for a qualified first aider to administer first aid and/or my child's prescription medication (with the prescription label attached only and clear instructions) if necessary. Please list anything above in the medical details part of form
- I give permission for photos and/or to be taken of my child and allow them to be shown without his/her name to be used in future marketing material by Surrey Performing Arts
- I would like to be added to the Surrey Performing Arts email newsletter list to be informed about future classes and workshops
