This Church Membership form is here to provide a way for a believer in the Lord Jesus Christ to express a definite commitment to this local body of Christ. This is for adults 18 & older. Youth who want to join must get their parents permission.
To become a member of a church is to formally commit oneself to an identifiable, local body of believers who have joined together for specific, divinely ordained purposes.
