Thank you for taking the time to fill out your Boys Statewide Campout pre-registration form online. A fee of $15 per person (excluding pastors) is required when submitting this form.
The deadline for early registration ends on April 10th. Registering by this date ensures a T-shirt for each registrant. Registrations will be accepted after this date, and extra T-shirts ordered will be given out on a first-come first-served basis. Orders for additional T-shirts will be fulfilled after the campout.
This year you will have the option to pay by credit card or to send a check. You may also pay the full registration amount of $60 by credit card or check with the submission of this form. These prices are outlined on the summary page of this form.
Once the form is filled out, you will be allowed to preview your answers and go back to edit them if needed. You will also be able to print a summary for your records and for your church treasurer if needed. Your submission will be automatically generated and a confirmation will be sent to the email address you provide. Be sure to click the "SUBMIT" button when you are satisfied everything is correct. You should receive a confirmation email.
