Please complete the form below to submit any alleged abuse or misconduct, including emotional, physical, sexual misconduct or abuse and MAAPPS violations and deviations. Reporting is free and can be done anonymously by entering "anonymous" in the name field.

By submitting this form, you are giving permission to USA Baseball Athlete Safety staff to contact you with regard to the alleged incident, and authorizing USA Baseball to share all information you provide with law enforcement authorities and other organizations at its sole discretion. The information you submit will also be used and shared as set forth in our Privacy Policy. .

USA Baseball encourages honest and effective reporting; knowingly making a false or vindictive report is unethical and will not be tolerated, and may be a violation of federal law.

All adults working with minor athletes are mandatory reporters under federal law, and if the law enforcement authorities have not yet been contacted, they should be contacted immediately.
