    Misc. excavations and test hole jobs for complex construction start with the letter E, all other jobs are 3 digit number job numbers FOR MAIN AND SERVICE TYPE WORK). OQ's will be verified for all Main and Service related work by National Grid with a separate report. you must choose the proper job type

    PLEASE NOTE THAT SELECTING YES STATES YOU WILL BE DONNING AND USING FRESH AIR BREATHING APPARATUS ACCORDING TO NATIONAL GRID GUIDLINES. ONCE YOU SELECT YES YOU WILL BE PROMPTED TO TAKE A PHOTO OF ALL REQUIRED MEMBERS WITH BREATHING APPARATUS AND FACE PIECE DONNED WITH ALL REQUIRED PPE AS WELL AS A PHOTO OF THE GUAGE ON THE AIR CYLINDER TO SHOW THAT IT IS FILLED TO THE PROPER LEVEL. THESE PHOTOS WILL AUTOMATICALLY BE SENT TO NATIONAL GRID CONTRACT OVERSIGHT REPRESENTATIVES.

  • Excavation Log

    All Excavations are to be evaluated by the competent person at least once per day, prior to entry and as needed throughout the shift - Estimate DEPTH based on prints & update prior to entry if needed. The excavation depth must be measure from the deepest point. Excavations 5 feet or greater require a protective system unless within stable rock
  • Soil Classifications: Each soil and rock deposit shall be classified by a competent person as Stable Rock, Type A, Type B, or Type C using OSHA approved visual and manual tests.  If, after classifying a deposit, the properties, factors, or conditions affecting its classification change in any way, the changes shall be evaluated by a competent person.  The deposit shall be reclassified as necessary to relfect the changed circumstances.  Brief Desciriptions of soil type can be found here -> Click here for brief Soil Type Descriptions.     Means of Egress: Ladders or other safe means of egress shall be provided in excavations 4' or greater and (1) per 25' lateral travel for employees.  Ladders used in excavations secured ad extended 3' above landing or grade level.

    Soil Setback: Soil and materials must be maintained 24" from the edge of the excavation.
