Photography/Media Release

As Parent and/or Guardian of the above named participant(s), I understand that photos or videos may be taken of my child(ren) at this event. By checking below, I hereby grant permission for images featuring my child(ren) to be used in future marketing materials and/or social media posts including website, facebook, instagram and print brochures, for the sole purpose of sharing about Creating Friends with the community and its supporters.

I understand that names and ages (or other identifying information) will not be published alongside or apart from any media on behalf of Creating Friends or its sponsoring organizations without prior and explicit consent given by me.

Informed Consent and Acknowledgement

I hereby give my approval for my child(ren) to participate, in as much as they are able and willing, at this year's Creating Friends event taking place on July 26, 2025 in Snohomish, Washington.

In case of injury, I hereby waive all claims against Creating Friends and its sponsoring churches (Trinity Church and Emmanuel Church), including all volunteers, participants, and visiting parties. I acknowledge that there is a risk of being injured inherent to all outdoor recreational activity, including contact with and handling of therapy animals, however trained. In the event that I prefer my child(ren) not to participate in any given activity, I will provide written documentation prior to or upon arrival at the event.
