*The term "Staff" in this Contract and throughout this form will refer to any adult participant up at Sierra Bible Camp.
While directors and staff are expected to exercise reasonable care, and while I am a voluntary, non-paid, staff member over 18 years of age, I agree not to hold them or the camp liable for any injuries or accidents incurred during camp or on the way to or from camp. I understand that Galt Lodi Bible Camp, and/or any other Leadership Provider assumes no financial obligation, but in case of illness or accident, the directors of the camp, or their designated representative, have my authority to secure necessary medical attention for myself and/or my child(ren), whom, by my presence up at camp with them, I am ultimately responsible for, while at Sierra Bible Camp or while being transported to/from Sierra bible Camp. Should it be deemed necessary for myself, or my child(ren) to receive medical care, I agree to take full responsibility for any expense involved. I further accept the responsibility that I am and that my child(ren) is/are physically and mentally fit to participate in the activities of Bible Camp, and that I am able and responsible(for myself and the children I am Guardian over) to avoid or abstain from any substance that might cause any allergic reaction to myself/ourselves . I have read and understand the camp behavior and dress code expectations (found on the camp information sheet) and agree that I will abide by them and will be responsible to see to it that my child(ren) abide by them as well. Furthermore, I understand that if I have not submitted my requested background check with enough time for the results to be returned to the camp director, I will not be able to participate as a staff member this year. As a staff member, I will comply if asked by the Camp Director, Camp Caretaker, or any other Directive Staff authorized by the Camp Director or Camp Board to resign from my previously assigned staff role and vacate the premises immediately upon being informed, and a reason can but does not have to be given. I give permission for any photos of myself or my child(ren) to be used in future posts, slideshows, on the camp website, etc. at the discretion of Galt Lodi Bible Camp, and/or Affiliates.
Final Disclaimer: Camp takes place in the mountains. There are bugs, bats, deer, bears, snakes, squirrels, plants, and many other living things. There is dirt; there are rocks, roots, tree branches, and campfires. We go swimming; we play sports, including some activities at night when it is more difficult to see. Kids and staff may fall down and may get hurt. While affiliates of camp would never purposely cause injury to a child or staff member, these things just happen sometimes. By signing my paperwork / child's paperwork and by sending my child to camp, bringing my child to camp, or attending camp myself, I acknowledge that a camp experience comes with certain risks of injury, and this is a risk I am willing to take while agreeing not to enter into a lawsuit against Galt Lodi bible Camp, Sierra Bible Camp, or any of its staff members or affiliates for injuries or illnesses sustained during the time of camp.

Galt Lodi Bible Camp
This background check is necessary because we are trusted with minor children at this camp. Thank you for taking a moment to fill it out. We appreciate it, and we know that you as a parent sending your child to a camp appreciate it as well.
I hereby authorize Galt Lodi Bible Camp/or its agents to make an independent investigation of my background, references, character, past employment, education, criminal, or police records, including those maintained by both public and private organizations and all public records for the purpose of confirming the information contained on my application and/or obtaining other information, which may be material to my qualifications as a volunteer or for employment now, and if applicable, during the tenure of my volunteering or employment with Galt Lodi Bible Camp.
I release Galt Lodi Bible Camp and/or its agents and any person or entity, which provides information pursuant to this authorization, from any and all liabilities, claims, or lawsuits in regards to the information obtained from any and all of the above referenced sources used.
$40 Per Participant (No Cost for Ages 2 and Under)

· Food To and From Camp

If you will be assisting us in providing camp arranged transportation you shall be asked to provide a copy of your current insurance to be placed on file.
It is expected that campers will behave with language, clothing, and conduct that reflects a bible camp atmosphere. No profanity, vulgar language, or inappropriate comments will be tolerated. There will be no smoking, chewing, drugs, (with the exception of prescriptions to be turned into the nurse), no alcohol in any form, no headphones, game devices, and if a camper is seen out of their cabin with a cell phone, it will be taken from them and given back at the end of the week. SBC, Galt Lodi Bible Camp, and affiliates are not responsible for any missing or damaged items.
As a Christian Camp, campers are required to wear modest, appropriate apparel. No crop tops, spandex shorts, spaghetti straps, large holes that allow undergarments to show, midriff showing or inappropriate pictures or logos. All shorts must have an inseam of AT LEAST 3 inches in length. A one piece bathing suit (females) (tank tops for males) is permitted with shorts during co-ed swim time. If a camper has a two piece, or a suit with low cuts or holes, they will have to cover with shorts and a camp appropriate shirt. If a camper is found wearing any clothing considered to be inappropriate outside of the cabin or restrooms, they will be asked to return to their cabin and change. Clothing for play and casual wear may be worn on all occasions. We do have a banquet night during which some campers like to dress up. This is not required. We strongly suggest labeling your campers belongings (clothing and towel tags) with permanent marker. Laundry service is not provided except for emergencies.
Suggested items to bring for weekend:
WARM Sleeping bag or blankets, (PLEASE CHECK THE WEATHER, IT CAN GET COLD AT NIGHT!) one sheet to cover camp mattress, pillow with pillow case, towels and washcloths, pajamas, warm jacket, sweatshirt, clothing, hiking shoes, pool/shower shoes, bathing suit with cover up, bug spray, sunscreen, chap-stick, flashlight, toiletries, BIBLE, notebook, writing materials, foldable camping chair, *Feel free to bring your guitar and the notes to your favorite worship song
Any Staff / Campers who will be staying the entire week, need to fill out the Staff or Camper Registration Forms separate from this form. They do not need to be included on this form.
Remember to pre-print the driving directions found at Lodibible.camp under the FAQ tab in case reception drops, a phone dies, etc.
Don’t forget, the after camp slideshow will be at 6pm, Sunday June 30th in Lodi. Mark your calendars! It is an amazing time to check back in with each other after camp, worship together, and share memories from the week with family and friends! – Exact Location to be announced
Please remember to cover camp in prayer: especially that our Savior is honored in all things and for the spiritual growth and the safety of all campers and staff.

Our only goal is to point young lives toward Jesus Christ our Savior and host an amazing week of growth and worship for our youth.
