By signing below, Vendor agrees to the following Disclosures & Procedures:
•Vendor understands that this completed form is not a guarantee of work
•Vendor understands that Shamrock Community & Property Management is not the contracting party (unless specifically noted) and that all services or products provided or delivered by vendor is to the contracting association or managed property
•Vendor understands that contracts with Homeowner Associations are executed by the Association and not Shamrock Community & Property Management
•Vendor agrees to provide updated information as applicable, including updated certificates of insurance
•Vendor understands that Shamrock Community & Property Management has thirty (30) days to submit all payments after submission of invoice, unless other arrangements have been agreed to beforehand
•Vendor is responsible for all insurance to protect products, services, equipment, etc. provided to contracting association or property
•Vendor agrees that all of its employees that work on its behalf at any Shamrock Community & Property Management managed property or association has been vetted for legal work status and are covered by workman’s compensation insurance
•Vendor agrees to send all invoices to the mailing address at 6107 SW Murray Blvd · No. 105 · Beaverton · OR · 97008OR via e-mail to ap@shamrockcommunity.com. Vendor understands that inovices sent to any other mail or e-mail address may not be paid in a timely manner
•Vendor agrees that all information provided in this form, including attachments, is correct and that Vendor has read through all of the above mentioned Disclosures and Procedures.
