We hope your child will be able to attend all five mornings, but to allow us to ensure no spaces are wasted please indicate below which days you hope your child will be able to attend:
I give permission for my child’s photograph to be taken by EBC Summer Holiday Club leaders/local newspaper reporter for use on the Ellon Baptist Church Facebook page, website or in the local newspaper.
I give permission for my child to take part in EBC Summer Holiday Club. EBC uses the above information to enable it to fulfil its safeguarding responsibilities. In the unlikely event of illness or accident, I give permission for any appropriate first aid to be given by the nominated first aider. In the event of a medical emergency, every effort will be made to contact a parent/carer, but relevant information will be passed to the medical emergency services should the need arise. If necessary, this may include hospital treatment including anaesthesia.
Data collected on this form will be used by the leaders of EBC Summer Holiday Club for the administration of the club, which may include contacting you with information related to the activities of the club, and for ensuring the safety and well-being of children in the club. This form will be stored securely and destroyed after the club ends. I grant permission for our personal data to be processed for these purposes.
