To ensure you receive the correct testing, please review the following criteria prior to completing this form.

Screening Mammogram

A screening mammogram is an annual exam for signs of breast cancer.

If you are not having any new problems and any of the following apply to you, please complete this form to schedule your screening mammogram.

Age 30 years or older
You are due for an annual exam (at least 1 year since last mammogram)
You have a family history of breast cancer
You have generalized or menstrual cycle-related breast pain
You have breast implants

Diagnostic Mammogram

A diagnostic mammogram is used to evauate any current abnormality or concern.

This mammogram requires a referral. If any of the following applies to you, please contact your Healthcare Provider for a referral and they will contact our office to schedule your exam.

You have a new lump
You have new breast discharge
You have breast pain that is new or specific to one area of your breast
You have been diagnosed with breast cancer in the last 7 years
