*If my foster needs to be seen by a veterinarian, I will get pre-approval from director

*If my foster animal is seen by a veterinarian, and I request any additional medical services without pre-approval, I assume responsibility of payment for said service.

*If I take my foster animal to an unauthorized rescue veterinarian for SALVAGED SOULS PET RESCUE, I assume responsibility of payment.

*I understand I am responsible for getting all documentation *(shelter paperwork, medical records, vet visits, etc., sent to director in a timely manner.

*I understand any information, vet visits, etc., are to be logged under SALVAGED SOULS PET RESCUE as the owner of my foster animal.

*I agree to keep director updated on the well being of my foster animal.

*If I want to be considered as an "adopter" for my foster, I agree to follow protocol in the standard adoption process for SALVAGED SOULS PET RESCUE, including paying the required adoption fee.

*If my foster dogs needs to be groomed, I agree to contact the director, so the foster dog can be scheduled at the Salvaged Souls Pet Rescue's place of choice, and under the name of SSPR.

*I will not take my foster dog to be shaved down, without consulting the director of SSPR.

*I understand my foster dog is not allowed to leave the DFW area, without approval from Salavaged Souls Pet Rescue director, or someone on the board of directors.

I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any SALVAGED SOULS PET RESCUE animal fostered by me.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless SALVAGED SOULS PET RESCUE and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.

I accept full responsibility for the dog(s) actions at all times, and release SALVAGED SOULS PET RESCUE from any liabilities or damages that may be incurred because of fostering such dog(s).

I agree to have SALVAGED SOULS PET RESCUE complete reference call checks and conduct a home visit inspection to be able to approve my foster application.

I agree that if I'm unable to foster the dog(s) anymore that I will return the dog(s) to SALVAGED SOULS PET RESCUE and try to give SALVAGED SOULS PET RESCUE a 2 week period to try and find another suitable foster for the dog(s).
