2019-2020 Fidelity Assurance Form
Introduction
The Fidelity Assurance Form must be submitted when a student needs to utilize one or both of the items listed below as tools or accommodations:
- Internet Access: Applies to instances when software, such as web-based Assistive Technology devices used by the student routinely and documented in the student’s plan, require an active connection to the internet.
- Software that contains prohibited features which cannot be disabled: Applies to instances when software, such as web-based speech-to-text applications used by the student routinely and documented in the student’s plan, contain prohibited features that CANNOT be disabled.
Requirements
- The tool or accommodation (as described above) must be part of the student’s Individualized Education Plan, Section 504 Plan, Individual Learning Plan, or Service Plan and used by the student routinely in the classroom.
- If the student is being tested in a small group setting a Proctor must be present in the testing room (along with a TA). The proctor must be next to/near the student to monitor the student to ensure the student is not accessing any content other than the assessment.
- If the student is being tested individually, only the TA is necessary and the TA should be next to/near the student to ensure the student is not accessing any content other than the assessment.
- At the conclusion of test sessions each day, the device must be examined to ensure that none of the test components or the student’s responses are stored on the device.
- For Computer-Generated Responses: Upon completion of a test, the TA must print the computer-generated responses on white paper, and then insert the pages in the student’s assessment book inside the front cover and follow the directions in the Test Coordinator’s Manual/Test Administrator’s Manual (TAM), for preparing materials for processing and scoring.
The Corporation Test Coordinator (CTC) is acknowledging understanding that:
- When software used by the student requires an active connection to the internet, the student will be monitored to ensure he/she remains ONLY within the necessary software (i.e. ensure the student utilizes no other websites or web-based tools) during the duration of the assessment.
- When prohibited features cannot be disabled, the student must be monitored to ensure that he/she does not access any prohibited features which cannot be disabled during the test administration.
- If the student is being tested in a small group setting a Proctor must be present in the testing room (along with a TA). The proctor must be next to/near the student to monitor the student to ensure the student is not accessing any content other than the assessment.
- If the student is being tested individually, only the TA is necessary and the TA should be next to/near the student to ensure the student is not accessing any content other than the assessment.
