1. Enrolling member must be 18 years of age or older.
2. Must have a valid email address and credit card to have on file for monthly payments.
3. Only one member per enrollment. Additional members must enroll seperately.

Payment Agreements:

By enrolling in the Wellness Program, you are:
- Granting Courtney Truax, LMT to email you an invoice for your program option to be paid on the 25th of the prior month of enrollment. (ie. June 25th bill for July enrollment).
- Providing a valid credit card to have on file for future automatic payments to be collected on the 25th of each month.
- Agreeing to give a written 14 day advanced notice of cancellation or changing program enrollment options.

After you submit for enrollment, you will be sent an invoice to your listed email address shortly.

Once you have made payment and provided a valid credit card to be stored on file, you will be ready to come in for your massages starting the 1st of the upcoming month!
