Memories By Design is proud to assist you and your family in the production of your Tribute Video that honors and celebrates your loved one's life. Before you get started on the completion of this order form, we suggest that you do a couple of pre-planning things first.
- Select the 45 photos you wish to use.
- Please number them in the order in which you would like them to appear (File name starting with 01, 02, 03, etc.)
If you have additional photos or want to include video clips, please contact your cremation service provider with that information, as additional charges may apply.

Can't find a specific song you would like to use? That may be because it is a licensed and copyrighted song. The music industry is very protective of its music, artists and community. In this case, a Synchronization (Sync) license is required. A synch license is an agreement between a music user and the owner of a copyrighted composition (song), that grants permission to mix the song with any sort of visual image (video, photos, slideshows, videogames, etc.)

For example: Whether you release a YouTube video or create a DVD of your band playing a Rolling Stones, Beach Boys or Frank Sinatra song, if you use only a portion of the song, you need a Synchronization License. A Sync License is required no matter how small a portion of the song you use. The cost of a license can cost into the thousands of dollars.

If you are not sure, Memories By Design will be happy to assist you. We can conduct a search for you, and if the music is licensed and copyrighted, we can attempt to contact music publisher on your behalf, to request permission to use of this music at no cost. Honestly, this is a longshot but we have had success. If the music is public domain, we will happily assist you in attaining the very best quality available.

Part 3 - Upload Files

Following the instructions:
1. Simply drag & drop your photos to the page or click on the computer icon/box to search for the files you want to add.
2. Complete the “From” section with your name and email address AND include your Cremation/Service Provider.
3. Add to “Message” is optional
4. Click ‘Upload’
Once your files are delivered, our team will send you an email confirming their receipt.
