Liability Weiver:

I, the undersigned, being aware  of my own/(child’s) health and physical condition, and having knowledge that my participation in any exercise program may be injurious to my health, am voluntarily participating in a physical activity. 

Having such knowledge I hear my knowledge this release any representatives, agents, and successors from liability for accidental injury or illness which I may incur as a result of participating in the said physical activity. I herby assume all risks connected there with and consent to participate in said program.

I agree to disclose any physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in said fitness program.

 Things to know:

You are responsible for your sessions.

If you miss a session without prior contact there will be no make up.

We ask for an open line of communication. 

In the event that a session is missed or canceled do to the trainer not being available then that session will be rescheduled 

Session maybe rescheduled if a tragic event happens or the unforeseen happens. We are all human and things happen but, “I was sore “ is not an excuse.

 

Payments:

All payments are expected on time. 

Payments will come out on the desired date scheduled at the time of sign up.

A 30 days notice is required before termination of contract! 

If you are signing a yearly membership there will be be a fee of $85 for early termination!

In the event that a payment is more than 5 days late (without prior notice) a late fee of $25 will be added! This also goes for payments that go into deferment.

Please understand that this is your financial obligation to your health and it needs to be taken seriously!
